Everyone who's over the age of 14 and on the learning disability register is entitled to a free annual health check.
Make sure their name is listed on the register by contacting your GP surgery.
Complete our checklist before a health check, to make sure you have all the information the GP needs:
Some people may not be able to communicate a new pain or sensation they’re feeling. They also might not fully understand how important it is to get these things checked out.
Use the checklist to help them understand what information the GP needs and why, so they can make an informed decision to share it. If they don’t have the capacity to understand, consider whether you’re acting in their best interest.
