Transition means changing from one state, stage, place, or subject to another and usually, moving on from being a child to becoming an adult.
For children and young people with special needs or disabilities it means going through the stages of moving from school into adulthood pathways, such as further education, employment or training. It's also the point at which a young person with SEND has to change from using children's services to adult care services.
From Year 9 a Preparing for Adulthood Transition Plan (PfA Plan) can be requested by young people that have SEND to help them prepare for their life post school. More information about each organisation’s role in the process is available from YC Hertfordshire.
