Here's the process for contacting the 0 - 25 Together Service and finding out if they can support you.

Neurodiversity Support Hub (ADHD and Autism)

For any queries or concerns you may have around Autism or ADHD.

Early Years Advice Line 

If you have concerns about your young child (0-5 years) and their development

Physical and neurological impairment team (PNI)

Speak to a specialist advisory teacher about concerns you have about your child's physical or neurological development and how this might impact their learning.

Education Support for Medical Absence (ESMA)


For queries about educational support if your child or young person is unable to attend school due to medical reasons.

Therapy services (including physiotherapy, occupational therapy and speech and language therapy)

Get advice from occupational therapists, physiotherapists and speech and language therapists about your child or young person.

01923 470680 – select Option 3 for the therapy service. Callers then choose to use the service Advice Line (select 1) or to speak to an administrator (select 2).

School Nursing Duty Line (for physical and emotional wellbeing)

The HCT school nurse duty line is available every weekday and can support you with any concerns you may have about your child or children's physical and emotional wellbeing.

Advice around exclusions and suspensions (Attendance Team)

A general queries and advice line for schools, parents and professionals for matters relating to school exclusion and suspension.

0 - 25 Together Service (social care)

If you want to find out if you can be assessed for social care support, call the customer service centre on 0300 123 4043. You will be asked some general questions about you and your family, and this information will be passed along to the service. They will take a look at your information and be in touch with you. Depending on your information, they may decide to assess, or they may provide you with information about support that could help you.

SENDIASS

SENDIASS provides an accessible service, signposting where appropriate, and offering impartial information and advice on matters relating to education, health and social care.  

They aim to ensure children with SEND and their parents, as well as young people with SEND, are supported and empowered to participate confidently and as fully as possible in all decisions about themselves and to have their voices heard.

If there is disagreement, SENDIASS promote opportunities for early resolution and support parents and young people to work together with the appropriate services to find an agreement and a positive way forward.
