Many young people find it easiest to complete with a teacher or support worker because they understand what help is available.
We designed this tool with young carers and the people who support them.
The tool is not a test. It helps us understand the care you provide, how often you provide it, and how it affects you. Once we understand your situation, we can discuss support for you and your family.
For example, helping teachers to understand why homework is difficult, finding more time for friends or hobbies, offering a Young Carers Assessment.
Young carers are entitled to have a Young Carers Assessment. Although it may sound daunting, it’s simply a way for you, your family and anyone you choose to tell us what is working well and what could be improved.
Some young carers get a lot of positives from their caring role, but if you want some support with your emotional wellbeing, visit HertsHub.
The form comes to us (Hertfordshire County Council) so we can offer support related to your caring role. This might include checking in with you, support with education, emotional wellbeing, opportunities for you to have a break from your caring role. These are the things young carers have told us are important to them.
Because caring roles can change, you can review or complete the form again every year or sooner if your caring responsibilities increase.
